RESUMO: ABSTRACT Objective: To identify variables and assess their effect size on orthodontic treatment duration of class III malocclusion. Material and methods: Forty six class III malocclusion cases were selected from 2008 patients’ records. Clinical charts and pre and post-treatment dental cast were evaluated. Age, sex, PAR index at T1, PAR index at T2, overjet, missing teeth, tooth extractions, number of treatment phases, missed appointments and orthodontic appliance breakages were investigated by multiple linear regression analysis and stepwise method at p<0.05. The sample was also divided in two groups, Group 0-2, including patients who had missed two clinical appointments or less and Group >2 with patients who missed more than 2 appointments, to detect the influence of this data on treatment time and the quality of finishing treatment (PAR T2). Results: Average treatment time was 31 months (SD=10.3). Missed appointments, brackets and bands breakages and PAR T2 were significantly associated to treatment length and responsible for around 56% of treatment duration. Age, sex, PAR T1, overjet, missed teeth, tooth extractions and one or two phase treatment had no significant effect on treatment time. Treatment of patients from Group >2 was nearly one year longer. However, no significant influence on PAR T2 was observed for those patients. Conclusion: Orthodontic treatment duration in class III patients is mainly influenced by factors related to patient compliance and orthodontic finishing. Patients who missed more appointment didn´t show worse orthodontic finishing, but longer treatment. Therefore, it is necessary alternatives to encourage patient cooperation during treatment. Other factors not included in this study may contribute to variability in treatment time.
